Personal Information Form

Sometimes referred to as a Family Template, this form provides a structured questionnaire that you can use to record each family group. You can use this form when you are conducting an interview because it has room to record all the important facts. Use a separate form for each family group or marriage. Family Tree Chart

This ancestor tree, sometimes called a pedigree chart provides a useful view of an individual's parents, grandparents for 4 generations. I find that limiting the form to 4 generations makes it easy for people to follow. I wanted to ensure that there was plenty of space for additional information and even stories to be recorded on the page. Research Log

This log is used by researchers to properly document and record all correspondence, interviews and research trips. This is perhaps the most underused form that family historians have access to. Using this form, before each interview or research trip will make sure that you know exactly what you are looking for.
